Mission and Vision Statements
Mission Statement: Our mission is to be committed to delivering compassionate, personalized care to individuals living with substance use disorders. Our approach integrates evidence-based medication and practices to reduce harm, promote recovery, and enhance long-term health outcomes. This will be done by fostering a patient-centered environment built on respect, trust, and empathy.
Vision Statement: In the future, our goal is to expand the practice so that our patients have the comprehensive care they need, as well as being more accessible to individuals who need MAT (medication-assisted treatment), primary care, mental health services, and overall assist as many people as possible.
Who
we are
Rebecca Diamond is family nurse practitioner who specializes in Family Medicine, Addiction Management, and Pain Management. She completed her Bachelor of Science in Nursing at Chamberlain University and then earned her Master of Science degree at Chamberlain University in Chicago, Illinois as a Family Nurse Practitioner. She holds a national board certification with the American Academy of Nurse Practitioners Credentialing Center (AANP) as a family nurse practitioner as well as completed her education in treating patients with substance abuse by Substance Abuse and Mental Health Services Administration (SAMHSA).
As a patient you can be expected to be greeted with a caring and professional attitude. You will get her undivided attention during your appointment. I will listen and understand the unique needs of each individual patient. I aim at prevention and promoting wellness. I work together with the patient on their treatment options and goals to help them achieve their best outcome possible.
